1. Secure online application fully completed and printed. (This form must be completed online at the Department of State web site and will print with a special bar code.)
   
   
2. Valid and signed US passport.
   
   
3. 2 recent passport size (2x2) photographs with white background.
   
   
4. Letter to the US Department of State, signed by APPLICANT, substantiating the need for a second passport. Sample Letter Template.

5. Authorization letter for release to Atlas Visa Service. (File size: 180 KB)
   
   
6. Copy of itinerary.
   
   
7. Processing fees (1-3 working days):
   
   Adults Age 16 and over: $170.00
   Minors Under Age 16: $140.00 + $30.00 Acceptance Fee to Post Office
   
   
8. Atlas Visa Service fee $125.00
   
   
9. Return Postage - $27.50 priority overnight (up to 3 passports) $37.50 (Saturday delivery)
