1. Secure online application fully completed and printed. (This form must be completed online at the Department of State web site and will print with a special bar code.)

3. Original proof of name change. (Court Documents or Marriage Certificate).
   
   
4. Authorization letter for release to Atlas Visa Service. (File size: 180 KB)
   
   
5. Copy of itinerary.
   
   
6. Processing time (1-2 working days);
   
   With passport less than 1 year old: $60.00
   With passport more than 1 year old, but unexpired: $170.00
   With expired passport: $170.00 + $25.00 Acceptance Fee to US Post Office
   
   
7. Atlas Visa service charges: $125.00 for rush.
   
   
8. Return Postage: $27.50 Priority overnight (up to 3 passports)
                                  FedEx Saturday Delivery: $37.50
