3. Original proof of name change. (Court Documents or Marriage Certificate).
   
   
4. Authorization letter for release to Atlas Visa Service. (File size: 180 KB)
   
   
5. Copy of itinerary for rush services.
   
   
6. Processing fees: $65.00 for rush - 1-2 working days.
   
   
7. Service charges: $125.00 for rush.
   
   
8. Return Postage: $24.50 Priority overnight (up to 3 passports)
                                  FedEx Saturday Delivery: $37.50
