1. Secure online application fully completed and printed. (This form must be completed online at the Department of State web site and will print with a special bar code.)
   

   -	Please do not sign the application until you take the oath administered by the passport acceptance agency.

   
   
2. Original proof of US citizenship.
   - Certified birth certificate or naturalization certificate or passport issued over 15 years ago
   
   
3. 2 recent passport size (2x2) photographs.
   
   
4. Authorization letter for release to Atlas Visa Service. (File size: 180 KB)
   
   
5. Copy of itinerary for rush services.
   
   
6. Processing Fee $135.00 plus $30.00- execution fee from the post office, 3-5 working days (processing time)
   
   
7. Completed Atlas Visa Service form. (File size: 213 KB)
   
   
8. Atlas Visa Service fee $125.00
   
   
9. Return Postage - $24.50 priority overnight (up to 3 passports)
   $37.50 (Saturday delivery)
